Abstract
Latar Belakang: Diabetes melitus tipe II dapat menyebabkan nefropati diabetik yang menurunkan fungsi ginjal secara progresif. Kreatinin serum dan estimated glomerular filtration rate (eGFR) merupakan parameter penting dalam deteksi dini gangguan fungsi ginjal. Tujuan: Mengetahui gambaran kadar kreatinin serum dan nilai eGFR pada penderita diabetes melitus tipe II di wilayah Puskesmas Telaga Dewa Kota Bengkulu tahun 2026. Metode: Penelitian deskriptif observasional dengan 30 responden menggunakan teknik accidental sampling. Kadar kreatinin serum diukur dengan metode Jaffe menggunakan spektrofotometer Zenix-188, sedangkan nilai eGFR dihitung menggunakan persamaan CKD-EPI 2021. Data dianalisis secara deskriptif dalam bentuk distribusi frekuensi dan persentase. Hasil: Sebagian besar responden berjenis kelamin perempuan (83,3%) dengan rata-rata usia 41,73 tahun (rentang 23–65 tahun). Kadar kreatinin serum menunjukkan 22 responden (73,3%) dalam kategori normal dan 8 responden (26,7%) di atas nilai rujukan. Nilai eGFR menunjukkan 14 responden (46,7%) dalam kategori normal (≥90 mL/menit/1,73 m²) dan 16 responden (53,3%) mengalami penurunan fungsi filtrasi ginjal (<90 mL/menit/1,73 m²). Kesimpulan: Sebagian besar responden memiliki kadar kreatinin serum normal, namun lebih dari separuh menunjukkan nilai eGFR abnormal. Hal ini mengindikasikan bahwa penurunan fungsi ginjal dapat terjadi meskipun kadar kreatinin belum meningkat signifikan, sehingga pemeriksaan eGFR secara berkala menggunakan persamaan CKD-EPI sangat penting sebagai standar pemantauan fungsi ginjal di fasilitas kesehatan tingkat pertama. Kata kunci: Kreatinin serum, eGFR, diabetes melitus tipe II, CKD-EPI. ABSTRACT Background: Type II diabetes mellitus can lead to diabetic nephropathy that progressively reduces kidney function. Serum creatinine and estimated glomerular filtration rate (eGFR) are important parameters in the early detection of renal dysfunction. Objective: To describe serum creatinine levels and eGFR values in type II diabetes mellitus patients at Puskesmas Telaga Dewa, Bengkulu City in 2026. Methods: A descriptive observational study involving 30 respondents selected by accidental sampling. Serum creatinine was measured using the Jaffe method with a Zenix-188 spectrophotometer, while eGFR was calculated using the CKD-EPI 2021 equation. Data were analyzed descriptively using frequency distribution and percentages. Results: The majority of respondents were female (83.3%) with a mean age of 41.73 years (range 23–65 years). Serum creatinine showed 22 respondents (73.3%) within the normal range and 8 respondents (26.7%) above the reference value. eGFR showed 14 respondents (46.7%) in the normal category (≥90 mL/min/1.73 m²) and 16 respondents (53.3%) with decreased renal filtration function (<90 mL/min/1.73 m²). Conclusion: Most respondents had normal serum creatinine levels; however, more than half showed abnormal eGFR values. This indicates that kidney function decline can occur even before creatinine rises significantly, making periodic eGFR assessment using the CKD-EPI equation essential for renal function monitoring at primary healthcare facilities.
